University Hospitals of Morecambe Bay NHS Foundation TrustDigital Outpatients

Over the last couple of years, UHMBT has focussed on transforming outpatient journeys from start to finish. Utilising multiple products from the Evolve Platform, the Trust has implemented a number of patient-centric end-to-end projects - such as online appointment bookings, digital waitlist screens, clinical forms and the digital patient portal. Putting patients back in charge of their care, the Trust has seamlessly integrated a combination of complex technologies to achieve a truly streamlined patient pathway.

Hertfordshire Community NHS TrustCYPT referrals

The Trust's fully automated CYPT referrals ensure children and young people access therapies as quickly as possible, reducing errors and wait times by replacing a previously paper-based process. Revolutionising a long-standing system in just 180 days, this award recognises HCT's dedication to some of its most vulnerable patients - utilising RPA for public good. The Trust also goes the extra mile in terms of sharing this project, allowing other healthcare organisations to find the same benefits through replicability.

Digital Project of the Year

Lancashire and South Cumbria NHS Foundation TrustDIALOG+

Though extremely complex, LSCFT's implementation of DIALOG+ makes it easier than ever to assess patient wellbeing and create effective recovery goals and interventions. Consisting of apps and eForms, the Trust's purpose-built system is informing practice while bolstering patient autonomy, giving access to personalised care and safety plans for clinicians and service users - improving experiences for both healthcare professionals and the public.

Home Group – Care and Support App

Providing supported housing to over 20,000 vulnerable people, Home Group have a responsibility to assess and support the needs of its tenants at all times. With over 1,000 dedicated Care and Support workers completing home visits and assessments, the team created a purpose-built app for use in the field. Allowing care workers to capture and retrieve back-office information with authorised mobile devices, the app allows workers to spend more time with tenants during visits, and less time capturing unstructured data on paper (before rekeying back at base).

Nottinghamshire Healthcare NHS Foundation Trust - Diary Caseload Allocation

As part of a wider EPR uplifting initiative, the Trust launched various projects involving RiO this year. Its Diary Caseload Allocation automation has saved over 58 working days so far - granting and removing permissions where necessary to ensure secure access to patient information where needed. A clear demonstration of Evolve's capabilities on a corporate licence scale, this project was just one of many that actively gave time back to healthcare professionals, so they can focus more on the patient-centric work that really matters.

Community Collaboration Award

Mid Yorkshire Teaching NHS Trust

Since beginning its automation journey only a couple of years ago, the Trust has become a leading pioneer in the NDL Community, continually leading public sector events to share the power of digital transformation within the NHS. Not only has the Trust shared its secrets to success as key industry speakers, but it has worked tirelessly to improve in-house skills through an NDL mentorship, before showcasing its achievements with a video case study. The Trust has demonstrated its willingness to share and collaborate with the NDL team and the wider public sector time and time again.
